Introduction
Personal Health Care provides service to infants with complex medical needs to older adults needing assistance with Activities of Daily Living and everything in-between.
Our Staff includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and Home Health Aides (HHA) whose focus is the one-on-one support of our clients and their needs. Personal Health Care has the ability to provide nursing or aide support for any of our client’s needs.
Personal Health Care serves their clients in Bucks County, Chester County, Delaware County, Montgomery County,Philadelphia County and the Mainline.
Personal Health Care provides comprehensive case management for every client. Our multidisciplinary healthcare team will assess the unique medical and personal needs of the client and create a plan that ensures safety and wellness while maintaining maximal independence and privacy. We assign one case manager to handle all of the client’s needs, increasing continuity, communication, and client satisfaction. Our case managers will ensure that the appropriate staff meets the needs of the client.
Most importantly , we take pride in our reliable and responsive staff who are available 24 hours a day, 7 days a week, 365 days a year.
