With a Physicians Order (DOH #4359) you can stay in your home, get the assistance you need, in your style, and on your schedule. Being Home, with a safe, engaging, and sustainable CarePlan, custom designed, for you, by you and your doctor.
You have complete ownership of the safety of your environment. We can assist you even better when you have a can-do attitude and own the true meaning and intent of Consumer Directed (“CDâ€) You take control to hire/fire, train, schedule, assign and manage your CD-Personal Assistant (“CD-PAâ€). We help you with a tool chest of specially designed CDPAS support services. . We advocate for you and assist in expediting your effort. We can help you navigate when you get stuck or challenged on your Consumer Directed Journey. We join your journey at whatever phase you are in, helping get things better for you and your loved ones.
At first, when you reach out to us, we listen to your concerns and most pressing CDPAS issues.
