Introduction
DocCare Medical Clinic established in 2016 is the primary care practice for adults over 16 and older. We provide comprehensive care Services including management of chronic diseases (like hypertension, diabetes, and high cholesterol), diagnosis and treatment of acute illnesses, and a wellness services (complete physical exams, immunizations, cancer screening, and lifestyle modifications). We provide experienced, knowledgeable and comprehensive care to help you to make you in usual Goals and to have a healthier tomorrow.
Our Physician is a well trained in internal medicine and has extensive experience in primary Medicine and Preventative Medicine. Our office does not have any nurse extenders or any physician assistants we believe that the care should be given by a certified Doctor of Medicine or Doctor Of Osteopathic Medicine. Our office is currently located on Hillsborough Avenue between 34 and 35th Street, opposite Dunkin’ Donuts. We have a new facility with modern and state-of-the-art equipment, with ample parking and easy access. We provide on-site minor surgical procedures and have a facility for intravenous fluid management.
Finally, we like to emphasize that we are an independent practice owned and operated by our physician. We believe that there should be a direct Physician and patient relationship without any corporation telling us how to manage our patients. At DocCare Medical Clinic, we have a contract with a group of hospitalists who will manage short stay or inpatient patient stay and in Skilled Nursing facilities. We are an independent facility that can make the decision that is best for you and your family and for us to provide you with a healthier tomorrow.

DR.Jayant Patel
MD, MBA
Dr. Patel is a specialist in the practice of Internal Medicine and has been a resident of Tampa over 30 yrs.
Dr. Patel did his medical education at Kasturba Medical College, and thereafter got his post-graduation training at Mount Sinai Hospital and Medical Center in Chicago, Illinois in Internal Medicine. He also completed Post-doctoral graduate training in Masters of Business Administration at University of South Florida Tampa. Dr. Patel has been involved with many health plans as a Medical Director and in the hospitals and has chaired many committees in the hospitals. He is married with 2 children and both of them are physicians.

Diabetic plan of care, Funduscopic Exam.
Pulmonary function test
Peripheral Vascular for vascular disease management
Minor surgeries, Intravenous fluid management.
Dermatologic procedures: Includes comprehensive skin service, biopsies, skin tag removal, wart removal, excision of lesions.
Women’s health examination and cervical cancer screening.
Vaccinations, Antibiotic injections, and Heart Failure management.
Coumadin clinic
On site blood draw.
