Concept Care is a registered NDIS provider specialised in providing in-home supports within the disability and home care sectors. Our aim is to be a pioneer in people-centric care and innovative support. We enable our participants to live independently, whilst empowering them to achieve their goals and dreams. Our diverse team of highly trained support staff, work diligently to ensure we consistently exceed expectations. We assist clients with activities of daily living (ADL), personal care (including high-care), domestic assistance, meal preparation, yard and garden maintenance, community access, travel and transport, respite/short-term accommodation (STA), day programs (recreational activities), companionship, 24-hour care, skill building (including budgeting, travel training etc.) and support coordination. Our service locations are Sydney, Melbourne, Brisbane, Adelaide, and Canberra.
